Step-1:

Create a new document 400x400 and fill it with color 919191.

Step-2:

Click Filter-Noise-Add Noise and set the following values.

Step-3:

Now go to Filter-Blur-Motion Blur. Type this settings:

Step-4:

Click OK and you are done.

Step-5:

Notice the ugly borders now? Use the crop tool and draw a square leaving those borders out. Right-Click and select Crop. Now they are gone.
